Supplemental Questions
  1. Please complete an essay describing your background, important influences in your life, and any achievements of which you are particularly proud. You may also address your education and career goals and how your disability has impacted you.
  2. What is your diagnosed disability or disabilities?
  3. Please provide documentation of your disability that is complete and up-to-date. The type of documentation needed depends on the nature of your disability. (Specific "Documentation Guidelines" are provided on the DSS web site in the Student Guide.)
  4. If you are an incoming student or transfer student, you must submit a certified transcript from the school you are currently attending.
